If you have questions or concerns regarding your insurance coverage, your family member's or patients  insurance coverage, please contact our billing office with your policy and group numbers.

Our Billing Center is open Monday through Friday from 8:30 am to 4:30 pm to provide patients, family members and medical professionals with the latest insurance regulations and qualifying coverage guidelines for their particular insurance carrier. We are a participating provider for Medicare and Blue Cross and accept all insurance plans containing ambulance coverage.  

Concord EMS complies with all Federal Regulations regarding patient confidentiality under the HIPPA Compliance Act.   Every patient receives our brochure which outlines those compliance standards. 

Patients being transferred from a hospital stay are required to obtain a statement from their physician regarding the medical necessity for transportation by ambulance. 

The Medical Necessity Authorization form can be obtained from the attending hospital staff or can be viewed and printed from the link below.
